PEMBERIAN ASI EKSKLUSIF MENGANDUNG BERBAGAI JENIS PROTEIN TINGGI
 UNTUK TUMBUH KEMBANG BAYI*

ABSTRACT
Exclusive breast milk is the behavior that provide breast milk only to infants up to six months without any additional food or liquid other than breast milk. Since giving exclusive breast milk has many nutrients one of which is protein. Digested into amino acids to accelerate the growth of the baby, a source of essential amino acids and non-essential, have antimicrobial activity so that they can contribute to the defense against viruses and bacteria pathogens, a comparison between Whey and Casein is one of the benefits of breast milk so the protein is more easily absorbed by the baby. Production of breast milk a day estimated to be between 750-1000 ml will energize 2100-2500 kilojoules or ± 600 kcal/liter and protein levels of 8.0 to 9.0 g/L.

ABSTRAK
Pemberian ASI eksklusif adalah perilaku yang memberikan Air Susu Ibu saja kepada bayi sampai berumur enam bulan tanpa tambahan cairan ataupun makanan lain selain ASI. Karena pemberian ASI eksklusif memiliki berbagai nutrisi salah satunya adalah Protein. Yang didigesti menjadi Asam Amino untuk mempercepat pertumbuhan bayi, sumber Asam Amino esensial dan non esensial, memiliki aktifitas antimikroba sehingga mampu memberi kontribusi pertahanan terhadap virus dan bakteri pathogen, memiliki perbandingan antara Whey dan Casein yang merupakan salah satu keunggulan ASI sehingga protein ASI lebih mudah diserap oleh bayi. Produksi ASI perhari diperkirakan antara 750-1000 ml yang akan memberikan energi 2100-2500 kilojoule atau ± 600 kkal per liter dan kadar protein 8,0-9,0 g/L.

PENDAHULUAN
Setiap orangtua pasti menginginkan bayinya lahir secara normal, sehat dan dapat tumbuh secara optimal, serta diharapkan menjadi manusia yang berkualitas dan berguna bagi masyarakat dan bangsa. Tugas mulia seorang ibu adalah hamil, melahirkan, kemudian menyusui bayinya. Sementara kewajiban orang tua adalah mendidik, membesarkan dan menjadi panutan bagi anak-anaknya agar impian mendapatkan anak yang berkualitas dapat terwujud.
Bayi baru lahir perlu mendapat perawatan yang optimal sejak dini, termasuk pemberian makanan yang ideal. Tidak ada satupun makanan yang ideal untuk bayi baru lahir selain ASI. World Health Organization (WHO) dan United Nations Children’s Fund (UNICEF) menganjurkan pemberian ASI secara eksklusif, yaitu perilaku yang memberikan ASI saja kepada bayi sampai berumur enam bulan tanpa tambahan cairan ataupun makanan lain selain ASI. karena ASI mengandung semua bahan yang diperlukan oleh bayi. Bayi baru lahir bisa memenuhi unsur penting untuk kekebalan tubuhnya hanya dari ASI (Wong, 1995).
Dalam kenyataannya, pemberian ASI eksklusif selama enam bulan. Banyak kendala yang timbul dalam upaya memberikan ASI eksklusif selama enam bulan pertama kehidupan bayi. Banyak sekali ibu yang meninggalkannya dengan alasan karir dan mengabaikan sang buah hati dengan susu formula. Data UNICEF menyatakan 30.000 kematian bayi di Indonesia dan 10 juta kematian anak balita di dunia tiap tahun bisa dicegah melalui pemberian ASI secara eksklusif selama enam bulan sejak kelahirannya. Akan tetapi tingkat kesadaran masyarakat untuk memberikan ASI kepada bayinya masih sangat memprihatinkan. Survei demografi kesehatan Indonesia pada 2002 menunjukkan pemberian ASI pada bayi satu jam setelah kelahiran menurun dari 8% menjadi 3,7%. Pemberian ASI ekslusif selama enam bulan menurun dari 42,2% menjadi 39,5%, sedangkan penggunaan susu formula meningkat tiga kali lipat menjadi 32,5% (Nuryati, 2008).
Padahal telah terbukti dari berbagai penelitian mendalam selama 50 tahun terakhir tentang komposisi dan keuntungan ASI. Air Susu Ibu selain memiliki susunan nutrisi yang ideal bagi bayi. Berdasarkan data, produksi ASI perhari diperkirakan antara 750-1000 ml yang akan memberikan energi 2100-2500 kilojoule atau ± 600 kkal per liter dan protein 8,0-9,0 g/L. (Picciano,2001, Wortthington and William 2000, Kretchmer and Zimmerman 1997) data dari Indonesia kadar protein 1,0 - 1,2 gram per 100 ml dan 60-69 kkal per 100 ml (Sediaoetama,2000).


PEMBAHASAN
Tidak ada satu pun orang tua yang ingin memiliki anak yang tidak cerdas, sakit-sakitan dan keburukan lainnya terhadap sang buah hati. Tapi tidak sedikit orang tua meremehkan akan hal pemberian ASI ekslusif pahadal kewajiban pemberian ASI ekslusif adalah ungkapan kasih sayang orang tua terhadap anaknya. Bahkan ungkapan kasih sayang Allah sekaligus anugerah yang luar biasa terhadap setiap bayi yang terlahir ke muka bumi, merujuk ayat-ayat tentang ASI. Antara lain :
“Para ibu hendaklah menyusukan anak-anaknya selama dua tahun penuh, yaitu bagi yang ingin menyempurnakan penyusuan. Dan kewajiban ayah memberi makan dan pakaian kepada para ibu dengan cara ma’ruf. Seseorang tidak dibebani melainkan menurut kadar kesanggupannya. Janganlah seorang ibu menderita kesengsaraan karena anaknya dan seorang ayah karena anaknya, dan warispun berkewajiban demikian. Apabila keduanya ingin menyapih (sebelum dua tahun) dengan kerelaan keduanya dan permusyawaratan, maka tidak ada dosa atas keduanya. Dan jika kamu ingin anakmu disusukan oleh orang lain, maka tidak ada dosa bagimu apabila kamu memberikan pembayaran menurut yang patut. Bertakwalah kamu kepada Allah dan ketahuilah bahwa Allah Maha Melihat apa yang kamu kerjakan” (Al-Baqarah [2]: 233)

Hikmah ayat yang terkandung dalam kitab Suci Alqur’an tersebut, setidaknya menekankan bahwa Air Susu Ibu (ASI) sangat penting untuk diberikan kepada sang buah hati. Komposisi kandungan penyusun ASI meliputi berbagai nutrisi antara lain kandungan energi, ion dan air, enzim, komponen seluler, karbohidrat, lemak dan salah satunya adalah protein. 

Kata protein berasal dari protos atau proteos yang berarti pertama atau utama. Protein adalah komponen terpenting sel hewan atau manusia. Oleh karena sel itu merupakan pembentuk tubuh kita, maka protein yang terdapat dalam makanan berfungsi sebagai zat utama dalam pembentukan dan pertumbuhan.  

Salah satunya, Protein merupakan sumber terbesar bagi bayi yang diperoleh dari air susu Ibu  yang terbentuk dari Asam Amino bebas di sel-sel sekresi dari grandula mamae. Pembentukan protein susu dan enzim mamae di induksi oleh prolactin dan dipacu insulin serta kortisol. Sintesa protein menggunakan Asam Amino esensial dan non esensial.

ASI mengandung berbagai jenis protein tinggi yang memberikan kualitas seperti yang telah dijabarkan di atas. Protein ASI didigesti menjadi Asam Amino untuk mempercepat pertumbuhan bayi, selain sebagai sumber Asam Amino esensial dan non esensial, terdapat beberapa protein lain yang memiliki aktifitas antimikroba seperti immunoglobulin, α kasein, lisosime, laktoferin, haptokorrin, α laktalbumin yang mampu memberi kontribusi pertahanan terhadap virus dan bakteri pathogen, hal tersebut telah ditunjukkan pada beberapa penelitian (lien. 2003)

Protein spesifik ASI dibagi menjadi 2 fraksi yaitu whey dan casein yang didasarkan pada perbedaan struktur fisik dan pola digesti. Casein menyusun 40 % dari total protein ASI yang disintesa linier bersama fosfoprotein di epitelium mamae dan berikatan dengan calcium, phosphate, dan magnesium. Kombinasi kasein dengan phosphate dan calcium membentuk miselium yang stabil yang disekresikan ke dalam ASI. Miseluim adalah bentuk yang efesien untuk asupan protein dan mineral pada bayi dan membawa jumlah lebih besar dari Ca, Mg, P dibandingkan bila dibawa dalam bentuk cairan air sederhana (Institute of medicine 1998, Kretchmer and Zimmarman, 1997)

Whey protein  terbagi dalam 3 bagian besar yaitu α laktalbumin 30 %, laktoferin 10-20 % dan imonoglobulin A 10 %. Sebagaian besar Whey dipecah dan merupakan sumber Asam Amino bagi bayi.
Adapun protein dengan peran berbeda yaitu  memiliki aktifitas prebiotic dengan meningkatkan pertumbuhan bakteri menguntungkan seperti laktobacilus dan bifidobakteri. Beberapa protein dan peptide memiliki aktifitas immunomodulator seperti sitokines dan laktoferin. (Institute of Medicine, 1997)

Pemecahan dan penggunaan zat gizi mikro dan makro juga dibantu beberapa jenis protein seperti lipase yang stimulasi oleh Asam empedu, amylase, β kasein, laktoferin, haptokorrin dan α1 antitripsin.

Peran lain protein ASI yaitu sebagi factor pertumbuhan insulin, epidermal dan laktoferin yang terlibat pada pengembangan dan pembangunan mukosa intestinal dan organ-organ lain dari bayi.
Laktoferin memiliki fungsi anti bakteri tertentu yang memerlukan besi, laktoferin juga memacu pertumbuhan dan pengembangan limfosit di usus bayi (Kretchmer and Zimmerman, 1997)
Kebutuhan protein bayi dan anak lebih besar dibandingkan orang dewasa. Angka kebutuhan protein bergantung pada mutu protein. Semakin baik mutu protein semakin rendah angka kebutuhan protein. Mutu protein bergantung pada susunan Asam Amino yang membentuk, terutama Asam Amino esensial dan ASI mengandung cukup protein yang bermutu. Kecukupan protein sehari untuk bayi (0-1 tahun) adalah 2,5 gram per kilogram berat badan (RS. Cipto Mangunkusumo dan Persegi, 1997)

Berdasarkan berbagai penelitian manfaat air susu ibu telah banyak dibuktikan Alarcon et al (1997) menyimpulkan bahwa pemberian ASI menurunkan frekuensi dan durasi ISPA akut dan diare pada anak-anak di bawah 6 bulan. ASI mempunyai efek proteksi yang kuat terhadap mortalitas anak (Shachdev, 1991)

Heing (2001), menganalisa berbagai penelitian berkaitan dengan efek lama menyusui dan kejadian kesakitan serta infeksi, dan disimpulkan bahwa ASI memberikan beberapa perlindungan secara fisik maupun biokimia. Terhadap agen-agen infeksi secara klinis di indikasikan ASI kemungkinan memberikan system imun pada bayi.


SIMPULAN
ASI mengandung berbagai jenis protein yang didigesti menjadi Asam Amino untuk mempercepat pertumbuhan bayi, sumber Asam Amino esensial dan non esensial, memiliki aktifitas antimikroba sehingga mampu memberi kontribusi pertahanan terhadap virus dan bakteri pathogen, memiliki perbandingan antara Whei dan Casein merupakan salah satu keunggulan ASI sehingga protein ASI lebih mudah diserap oleh bayi.

THE NATURE OF LIFE



The Nature of Life

have you ever dropped a pellet of dry ice (frozen carbon dioxide) into a pan of water and watched what happens? The solid pellet darts randomly about the surface, looking like a highly ener- getic bug waterskiing, as the warmer water rap- idly converts it to a gas. Does all that motion make the dry ice alive? Hardly; yet one of the attributes of living things is the capacity to move. But if living things move, what about plants? If a tree remains fixed in one place and doesn’t crawl down the sidewalk, does that mean it isn’t alive? Again the answer is no, but these ques- tions do serve to point out some of the difficulties encoun- tered in defining life. In fact, some argue that there is no such thing as life—only living organisms—and that life is a concept based on the collective attributes of living organisms.

ATTRIBUTES OF LIVING ORGANISMS
Composition and Structure
The activities of living organisms originate in tiny structural units called cells, which consist of cytoplasm (a souplike fluid) bounded by a very thin membrane. All living cells contain genetic material that controls their development and activities. In the cells of many organisms, this genetic mate- rial, known as DNA (an abbreviation for deoxyribonucleic acid), is housed in a somewhat spherical structure called the nucleus, which is suspended in the cytoplasm. In bacteria and other simple cells, however, the DNA is distributed directly in the cytoplasm. The cells of plants, algae, fungi, and many simpler organisms have a cell wall outside of the membrane that bounds the cytoplasm. The cell wall pro- vides support and rigidity.

Growth
Some have described growth as simply an increase in mass (a body of matter—the basic “stuff” of the universe), usually accompanied by an increase in volume. Most growth results from the production of new cells and includes variations in form—some the result of inheri- tance, some the result of response to the environment. What happens, for example, if you plant two apple seeds of the same variety in poor soil but don’t give them the same care? If you water one just enough to allow it to germinate and grow, while you water the other one freely and work fertilizers and conditioners into the soil around it, you might expect the second one to grow larger and produce more fruit than the first. In other words, although the two apple trees grew from the same variety of apple seed, they differ in form, following patterns of growth dictated by the DNA and the environment. 

Reproduction
Dinosaurs were abundant 160 million years ago, but none exist today. Hundreds of mammals, birds, reptiles, plants, and other organisms are now listed as endangered or threat- ened species, and many of them will become extinct within the next decade or two. All these once-living or living things have one feature in common: it became impossible or it has become difficult for them to reproduce. Reproduction is such an obvious feature of living organ- isms that we take it for granted—until it no longer takes place. When organisms reproduce, the offspring always resemble the parents: guppies never have puppies—just more guppies—and a petunia seed, when planted, will not develop into a pineapple plant. Also, offspring of one kind tend to resemble their parent more than they do other individuals of the same kind. 

Response to Stimuli
If you stick a pin into a pillow, you certainly don’t expect any reaction from the pillow, but if you stick the same pin into a friend, you know your friend will react immediately (assuming he or she is conscious) because responding to stimuli is a major characteristic of all living things. You might argue, however, that when you stuck a pin into your house plant, nothing happened, even though you were fairly certain the plant was alive. You might not have been aware that the house plant did indeed respond but in a manner very different from that of a human. Plant responses to stimuli are generally much slower than those of animals and usually are of a different nature. If the house plant’s food-conducting tissue was pierced, it probably responded by producing a plugging substance called callose in the affected cells. Some studies have shown that callose may form within as little as 5 seconds after wounding. Also, an unorganized tissue called callus, which forms much more slowly, may be produced at the site of the wound. 

Metabolism
Definitions of metabolism vary somewhat but are mostly based on the observation that metabolism is the collective product of all the biochemical reactions taking place within an organism. All living organisms undergo various meta- bolic activities, which include the production of new cyto- plasm, the repair of damage, and normal cell maintenance. The most important activities include respiration, an energy-releasing process that takes place in all living things; photosynthesis, an energy-harnessing process in green cells that is, in turn, associated with energy storage; digestion, the conversion of large or insoluble food mole- cules to smaller soluble ones; and assimilation, the conver- sion of raw materials into cytoplasm and other cell substances. 

Movement
At the beginning of this chapter, we mentioned that plants generally don’t move from one place to another (although their reproductive cells may do so). This does not mean, however, that plants do not exhibit movement, a universal characteristic of living things. The leaves of sensitive plants (Mimosa pudica) fold within a few seconds after being disturbed or subjected to sudden environmental changes, and the tiny underwater traps of bladderworts (Utricularia) snap shut in less than one-hundredth of a sec- ond. But most plant movements, when compared with those of animals, are slow and imperceptible and are mostly related to growth phenomena. They become obvi- ous only when demonstrated experimentally or when shown by time-lapse photography. Time-lapse photo- graphy often reveals many types and directions of motion, particularly in young organs. Movement is not confined to the organism as a whole but occurs down to the cellular level. For example, the cytoplasm of living cells constantly flows like a river within cells; this streaming motion is called cyclosis, or cytoplasmic streaming. Cyclosis usually appears to run clockwise or counterclockwise within the boundaries of each cell, but movement may actually be in various directions.

Complexity of Organization
The cells of living organisms are composed of large num- bers of molecules (the smallest unit of an element or com- pound retaining its own identity). Typically there are more than 1 trillion molecules in a single cell. The molecules are not simply mixed, like the ingredients of a cake or the concrete in a sidewalk, but are organized into compart- ments, membranes, and other structures within cells and tissues. Even the most complex nonliving object has only a tiny fraction of the types of molecules of the simplest living organism. Furthermore, the arrangements of these molecules in living organisms are highly structured and complex. Bacteria, for example, are considered to have the simplest cells known, yet each cell contains a mini- mum of 600 different kinds of protein as well as hundreds of other substances, with each component having a spe- cific place or being a part of a specific structure within the cell. When flowering plants and other larger living objects are examined, the complexity of organization is over- whelming, and the number of molecule types can run into the millions.
